Job Title: NDIS Support Worker
Position Overview: As an NDIS Support Worker, you will play a crucial role in providing support and assistance to individuals with disabilities, enabling them to achieve their goals and lead fulfilling lives. Your responsibilities will involve implementing person-centered care plans, fostering independence, and promoting social inclusion.
Key Responsibilities:

  1. Client Support:
    • Provide direct support to NDIS participants in accordance with their individualized care plans.
    • Assist individuals with daily living activities, personal care, and mobility as needed.
    • Foster a positive and supportive environment, promoting the autonomy and dignity of each participant.
  2. Goal Implementation:
    • Work collaboratively with participants to identify and set achievable goals.
    • Support individuals in reaching their goals by implementing strategies outlined in their NDIS plans.
    • Regularly assess progress and adjust support strategies as necessary.
  3. Community Engagement:
    • Facilitate community participation and social inclusion for participants.
    • Plan and organize community outings and activities based on participants' interests and preferences.
    • Encourage and support individuals to build connections within their local community.
  4. Documentation and Reporting:
    • Maintain accurate and up-to-date records of participant interactions, progress, and any incidents.
    • Prepare reports and documentation as required by the NDIS and organizational policies.
    • Collaborate with other team members to ensure the coordination of support services.
  5. Communication:
    • Communicate effectively with participants, their families, and other stakeholders.
    • Provide clear and concise information on available services, progress, and any relevant updates.
    • Actively participate in team meetings and training sessions.
  6. Qualifications and Skills:
    • Certificate III or IV in Disability Support, Individual Support, or a related field.
    • Experience in providing support to individuals with disabilities.
    • Knowledge of the NDIS framework and its principles.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Empathy, patience, and a genuine commitment to the well-being of individuals with disabilities.
  7. Working Conditions:
    • This position may require flexible working hours, including evenings and weekends.
    • Travel to participants' homes and community locations is a key component of the role.
